1. Documentation should be done (in SAP) as per the physician’s order and not authorized to do any documentation without the knowledge of the physician.

2. Documentation of the outpatient care in SAP

2.1. Transcribing / entering the outpatient management details in the SAP as per the physician conversation, dictated file or written format.

2.2. Transcribing the dictated below mentioned reports

2.2.1. Procedure / operation note

2.2.2. Discharge summary

2.2.3. Medical certificate

2.2.4. Consultation reports

3. Work edited and proof read to ensure accuracy

4. Spelling and grammar errors are minimized

5. Patient information identified on all reports.

6. Patient confidentiality maintained at all times.

7. Participation in quality improvement activities.

8.Ensure TAT is maintained at all time
